Assessment 4: Patient, Family, or Population Health Problem Solution

The current global conditions characterized by the COVID-19 pandemic have significantly influenced and disrupted the provision of healthcare services with specific reference to elderly patients. This intervention focuses on a 65-year-old female patient with COVID-19, and improve this patient's care using telehealth, community health assets, and education. This is an overarching strategy that targets this patient because she has got poor access to technology, finds it hard to navigate her healthcare needs, and will require adequate support systems. Because of the integration of prosthesis with other technologies and support from the community, the current intervention aims at enhancing the general wellbeing of the patient. Problem Focus The main scenario is the care for a patient, a 65-year-old female, with COVID-19 positive diagnosis. It would appear that this patient has several major issues in her context these includes coping with the symptoms at home; availing medical consultation at appropriate times; and getting appropriate care for other orphans needs. Her level of disability is still relatively high and the lack of technological literacy as well as social support makes the complexity of a one- dimensional intervention obvious. The patient's circumstances are caused by isolation as a result of implementing the social distancing policies adopted to prevent the spread of Covid 19 and may find it challenging to access traditional healthcare services hence the need to fashion out a workable solution that would overcome all these barriers. Relevance to My Professional Practice The significance of this problem to the professional practice is therefore very profound. But for healthcare provider admin and the treatment of elderly COVID-19 patients is a question
